HOW IS AORTIC STENOSIS TREATED?

No known medications slow the progression of aortic stenosis. If you have mild aortic stenosis, your cardiologist will monitor it with regular heart ultrasounds. Once symptoms appear or are severe on ultrasound, treatments include aortic valve surgery or transcatheter aortic valve replacement.

For patients with mild to moderate aortic stenosis or those who are asymptomatic, the initial approach often involves careful monitoring and conservative management. This strategy includes regular check-ups with a cardiologist, periodic echocardiograms to assess the progression of valve narrowing and management of other cardiovascular risk factors. Patients are educated about the symptoms of aortic stenosis progression and advised to report any new or worsening symptoms promptly. In some cases, medications may be prescribed to manage related conditions such as high blood pressure or to alleviate symptoms, although it’s important to note that there are no medications that can reverse the narrowing of the aortic valve.

Surgical aortic valve replacement (SAVR) has been the best way to treat severe aortic stenosis with symptoms for a long time. The blocked aortic valve is removed, and either a mechanical or biological prosthetic valve is put in its place during this open-heart surgery. SAVR is typically performed under general anesthesia and requires the use of a heart-lung machine. The choice between mechanical and biological valves depends on factors such as the patient’s age, overall health, and ability to take long-term anticoagulation medication. While SAVR is a major surgery with a significant recovery period, it has proven to be highly effective in improving symptoms and extending life expectancy for patients with severe aortic stenosis.

In recent years, transcatheter aortic valve replacement (TAVR) has emerged as a less-invasive alternative to SAVR for many patients. This procedure involves delivering a collapsible replacement valve to the heart through a catheter, typically inserted through the femoral artery in the groin. Once in position, the new valve is expanded, pushing the old valve leaflets aside and taking over the job of regulating blood flow. TAVR was initially approved for high-risk or inoperable patients, but its use has expanded to include intermediate and even low-risk patients in many cases. The minimally invasive nature of TAVR often results in shorter hospital stays and quicker recovery times compared to SAVR.

In some cases, particularly as a bridge to more definitive treatment or for temporary symptom relief, balloon aortic valvuloplasty may be considered. This procedure involves inserting a balloon catheter into the narrowed aortic valve and inflating it to stretch the valve opening. While this can provide temporary improvement, the valve typically narrows again within a few months to a year. As such, balloon valvuloplasty is generally not considered a long-term solution for most adult patients with aortic stenosis, but it may be useful in certain clinical scenarios or in pediatric patients.
